Ending the HIV Epidemic – Los Angeles

Since its founding in 1969, The Los Angeles LGBT Center has cared for, championed and celebrated LGBTQIA+ individuals and families through programs, services and global advocacy across four categories: Health, Social Services and Housing, Culture and Education and Leadership and Advocacy. The Los Angeles LGBT Center and APLA Health are joining forces to support the Los Angeles County Department of Public Health in achieving its Ending the Epidemic plan by improving and increasing coordination efforts between the county and HIV/AIDS service stakeholders, as well as increasing community engagement and advocacy efforts.

Foothill AIDS Project

Enhanced Continuum of Health Opportunities for HIV Care Linkage Program

The Foothill AIDS Project aims to improve the lives of those affected by HIV through comprehensive support services, through providing HIV outreach, education and testing to at risk populations, and link people who may be at increased risk of HIV to prevention medication. This program seeks to serve people living with HIV who are newly diagnosed, not virally suppressed and disengaged in care by supporting them with an enhanced continuum of support addressing barriers for retention in care, including behavioral health and housing insecurity.

East Bay Getting to Zero

Advance health equity and promote healing for all people impacted by HIV

East Bay Getting to Zero (EBGTZ) is a collaborative, community-driven initiative that improves access and eliminates barriers to HIV prevention and care in the East Bay region of Northern California. EBGTZ plays the unique role of bringing together the necessary leaders across community organizations, clinics, hospitals, schools, faith-based organizations, public and private agencies in Alameda and Contra Costa Counties to strategically implement innovative and evidence-based practices across the region.

Pacific Pride Foundation

Apricot Data Management System and Implementation

The Pacific Pride Foundation provides free or low-cost services to the HIV/AIDS and LGBTQIA+ communities, as part of ongoing advocacy and education efforts to meet the needs of the diverse population in Santa Barbara. Through a collaboration with Diversity Collective Ventura County, the Pacific Pride Foundation is working to implement an updated data management system to help improve tracking of demographics and health outcomes for all HIV prevention programs in Santa Barbara and Ventura counties.
